Cause of explosion at Philip Services determined accidental

KALAELOA (KHNL) - Investigators say a welder accidentally caused the explosion at Philips Services in Campbell Industrial Park that killed him.

The accident occurred while the worker, 23-year-old Sean Norva, was welding part of a catwalk that was connected to the top of an oil storage tank. 

The tank was about 15 feet tall and reportedly contained waste oil.  Vapors from the tank were ignited by welding activities and caused an explosion that fractured the tank and propelled it about 30 feet into a sand pile at an adjacent business. 

Norva was hurled over 100 feet by the blast and died at the scene.

Two other workers who were near the base of the tank when it exploded were seriously injured. 

A fourth worker at an adjacent business survived the blast with minor injuries.
