NTSB releases preliminary report on fatal Kauai ultralight crash

Investigators with the National Transportation Safety Board have released the findings of a preliminary investigation into a fatal light aircraft crash on Kauai earlier this month. 

The report indicates that the aircraft being piloted by 54-year-old Gerry Charlebois had recently experienced fuel system problems. 

Mark McKenzie, 53, of Ontario, Canada was also killed in the crash. 

Charlebois was the owner of Birds in Paradise, a powered hang glider school and tour company on Kauai, and had thousands of hours of experience operating ultralight aircrafts. He was also a FAA-certified Flight Instructor and mechanic.

Investigators say they are still looking at the wreckage, along with a camera that was recovered at the crash site.
