Pilot and air traffic controller error likely caused plane crash

HONOLULU (HawaiiNewsNow) - It's been a year since a plane crash killed a father and son in East Oahu and now federal investigators have released the probable cause of the accident.

The National Transportation Safety Board blames pilot error as well as the lack of warning from an air traffic controller.

According to the report, Dr. Nicholas Palumbo was flying in weather conditions that required the use of instruments but he was only qualified for visual navigation.

The NTSB also found that the air traffic controller failed to issue a safety alert after noticing the plane was heading toward the mountains.

The Honolulu veterinarian and his 20-year-old son, Tim, died when their Piper Cherokee plane crashed into a ridge last January.
